We are currently seeking a dedicated Physical Therapist (PT) who is passionate about enhancing patients’ quality of life through individualized home health care and wants to make a meaningful impact as part of our team
Duties
- Conduct comprehensive evaluations of patients’ physical condition, including physiology and anatomy knowledge, to determine appropriate treatment strategies.
- Develop detailed care plans tailored to individual patient needs, incorporating discharge planning and progress tracking.
- Provide initial and ongoing comprehensive assessment of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Deliver evidence-based physiotherapy interventions such as trigger point therapy, orthopedics techniques, and geriatrics-focused exercises to improve mobility and reduce pain.
- Instruct patient in care and use of wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
- Consult and familiarize staff regarding the established plan.
- Supervise Physical Therapy Assistants and home health aides as per agency policy.
- Record and report to the physician the patient’s reaction to treatment and/ or any changes in the patient’s condition.
- Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless transitions between care settings.
- Utilize electronic medical records (EMR systems) accurately to document assessments, treatments, and patient progress.
- Educate patients and caregivers on physiotherapy techniques, injury prevention, and health maintenance strategies to foster long-term wellness.
Requirements
- Valid licensure as a Physical Therapist with current registration in the relevant jurisdiction.
- Proven experience in home & community care environments with a strong understanding of home health protocols.
- Hands-on experience with acute care, inpatient hospital settings, outpatient clinics, or pediatric care is highly desirable.
- Knowledge of medical terminology, physiology, anatomy, and specialized therapies such as trigger point therapy.
- Proficiency in EMR systems for documentation and communication purposes.
- Strong skills in discharge planning, orthopedics, sports medicine, and outpatient therapy techniques.
- Minimum of one (1) year of institutional/ home health experience.
- Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
Pay: $75.00 – $90.00 per hour
