Health Promotion & Diabetes Wellness Coach

We are seeking an experienced Health Promotion & Diabetes Wellness Coach to support health and wellness initiatives that empower members to adopt healthier lifestyles and improve long-term health outcomes.

This role provides telephonic and program-based coaching, health education, wellness counseling, and disease prevention support using evidence-based behavior change strategies. The ideal candidate will have strong experience in health promotion and must possess direct diabetes coaching, education, or disease management experience.

The successful candidate will collaborate with internal and external partners, evaluate program effectiveness, and deliver structured diabetes self-management support designed to improve member engagement, health outcomes, and quality of life.

Key ResponsibilitiesHealth Promotion & Wellness Coaching

  • Conduct health assessments through telephonic outreach, health risk appraisals, and other wellness initiatives.
  • Provide health education and behavior change counseling to support healthy lifestyle adoption.
  • Coach members on preventive health guidelines and wellness topics, including nutrition, physical activity, stress management, smoking cessation, cholesterol management, and hypertension prevention.
  • Utilize motivational interviewing and evidence-based behavior change techniques to facilitate sustainable lifestyle changes.
  • Develop individualized goals, action plans, and strategies to support member success.
  • Research and communicate best practices in health promotion and disease prevention.
  • Prepare and deliver presentations, workshops, and targeted wellness programs for designated populations.
  • Collaborate with community resources, healthcare providers, and internal stakeholders to support member health initiatives.
  • Monitor, evaluate, and report on program outcomes and effectiveness.
  • Ensure compliance with applicable regulatory and accreditation standards, including NCQA, CMS, URAC, and HEDIS requirements.

Diabetes Management & Coaching (Required)

  • Provide structured diabetes education and coaching for members with Type 1, Type 2, and prediabetes conditions.
  • Support diabetes self-management, including glucose monitoring, medication adherence, nutrition planning, physical activity, stress management, and complication prevention.
  • Use motivational interviewing and coaching techniques to help members establish and achieve personalized health goals.
  • Coordinate referrals to healthcare providers, diabetes programs, and community resources as appropriate.
  • Document assessments, care plans, interventions, and member interactions accurately and within required timelines.
  • Track member progress and contribute to quality improvement and outcome measurement initiatives.

Required QualificationsEducation

  • Bachelor’s degree in Health Education, Health Promotion, Nutrition, Nursing, Exercise Physiology, Social Work, or a related healthcare field.
  • Master’s degree preferred.

Certifications

One or more of the following certifications is strongly preferred:

  • Certified Health Education Specialist (CHES)
  • Registered Nurse (RN)
  • American College of Sports Medicine (ACSM) Certification

For diabetes-focused responsibilities, the following are highly preferred:

  • Certified Diabetes Care and Education Specialist (CDCES)
  • Certified Diabetes Educator (CDE)

Licensure

  • Current, unrestricted Michigan professional license required for licensed disciplines (e.g., RN).
  • Valid Michigan driver’s license if travel is required.

Experience

Required:

  • Minimum of 2 years of experience in health promotion, health education, wellness coaching, or population health.
  • Minimum of 2 years of direct diabetes education, diabetes coaching, chronic disease management, or diabetes care coordination experience.

Preferred:

  • Experience working within managed care, health plans, disease management, or population health programs.

Knowledge, Skills & Abilities

  • Strong knowledge of diabetes management, disease prevention, and chronic condition coaching.
  • Demonstrated experience applying motivational interviewing and behavior change theories.
  • Ability to develop individualized action plans and support sustainable behavior change.
  • Knowledge of nutrition, exercise, weight management, stress reduction, smoking cessation, hypertension, and cholesterol management.
  • Excellent verbal, written, and interpersonal communication skills.
  • Ability to effectively coach and educate members telephonically and in group settings.
  • Strong organizational skills with the ability to manage multiple priorities independently.
  • Experience planning, implementing, and evaluating wellness and disease management programs.
  • Proficiency with Microsoft Office Suite and electronic documentation systems.
  • Knowledge of healthcare quality and regulatory standards, including NCQA, CMS, URAC, and HEDIS.

Preferred Candidate Profile

The ideal candidate is passionate about improving health outcomes, skilled in behavior change coaching, and experienced in supporting individuals living with diabetes. They are comfortable working independently, building relationships across teams, and using evidence-based approaches to help members achieve meaningful health improvements.

Pay: $35.00 – $37.00 per hour
