We are seeking an experienced Physical Therapist for a 13-week contract assignment in Hugo, Colorado. This is a unique multi-modality role supporting outpatient therapy, acute care, and home health services across nearby facilities within a 60-mile radius. Mandatory Requirements to Apply
- Active Physical Therapist (PT) license in Colorado or compact eligibility
- Minimum 1 year of recent Physical Therapist experience
- Must have outpatient physical therapy experience
- Experience treating post-operative patients, sports injuries, and geriatric populations
- Experience in both outpatient and inpatient settings preferred
- Strong patient assessment, treatment planning, and documentation skills
- Ability to travel between local facilities and home health locations within a 60-mile radius
- Current BLS certification preferred
Roles & Responsibilities
- Evaluate, develop, and implement individualized treatment plans for patients across outpatient, acute care, and home health settings
- Provide rehabilitation services for orthopedic, post-surgical, sports injury, and geriatric patients
- Monitor patient progress and modify treatment plans to achieve optimal outcomes
- Collaborate with physicians, nurses, and interdisciplinary healthcare teams
- Maintain accurate documentation and ensure compliance with facility and regulatory standards
Bonus & Benefits + Salary
- Pay Rate: $65–$69 per hour
- 13-week contract assignment
- Schedule: Monday–Friday, 8-hour day shifts (4×10 schedule may be considered)
