Job Details / Responsibilities:
Perform thorough patient assessments, observe and evaluate treatment effects, reassess needs, and report progress or recommend changes to the ordering physician
Formulate individualized treatment plans and coordinate services with patients and healthcare providers
Provide education to patients and caregivers on therapy goals, processes, and use of devices
Ensure accurate and timely submission of patient charges
Maintain strict patient confidentiality in all situations
Participate in professional development activities and maintain applicable affiliations and certifications
Attend required organizational meetings and committees
Perform additional related duties as assigned
Qualifications:
Doctorate in Physical Therapy (DPT) from a CAPTE-accredited program required
Active Indiana Physical Therapist license (credentialed by the Indiana Physical Therapy Committee)
Current BLS certification required
2–4 years of prior experience in a physical therapy role preferred
Strong clinical reasoning, communication, and patient education skills
Ability to work in a fast-paced clinical environment with physical demands of the role
