- Certification (Fundamentals of Coaching Education, Concussion, Sudden Cardiac Arrest, Heat Illness Prevention, Bullying, Hazing and Inappropriate Behaviors)
- CPR/First Aid Certification (Must have valid CPR & First Aid Certificate)
- Letter of Introduction
- Letter(s) of Recommendation (Must Submit Two (2) Letters Dated within the last Twelve (12) months.)
- Proof of HS Graduation (or equivalent)
- Resume
