Niagara Falls Memorial Medical Center is in growth mode and currently looking for Clinical Associates to join our Health Home team!
This position provides community coordination and management of Health Home enrollees under the direction of a Clinical Manager and the Health Home Director.
The position is responsible for engagement, coordination of all clinical, psycho social aspects of care to clients of the Health Home and collaborates with the clients, caregivers, physicians, and community-based programs. Care coordination occurs within the community setting.
Education:
- A Bachelor’s degree in any of the following: child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing; OR
- NYS licensure and current registration as a Registered Nurse and a Bachelor’s degree; OR
- A Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR
- A Credentialed Alcoholism and Substance Abuse Counselor (CASAC).
Experience:
- Two years’ experience (a Master’s degree in a related field may substitute for up to one year of experience) either:
a. Providing direct services to persons with serious mental illness OR
b. Developmental disabilities, or alcohol and/or substance abuse; OR
c. Linking persons who have serious mental illness or developmental disabilities, or alcohol and/or substance abuse to a broad range of services essential to successfully living in a community setting.
- 1-7 years experience in community health.
Requirements:
1. Niagara and Erie County travel using personal vehicle
2. A valid driver’s license and the ability to be insured by Niagara Falls Memorial Medical Center’s Carrier
3. Experience working with chronic disease including mental illness, substance use, medically fragile, and HIV/AIDS individuals
4. Ability to accurately document clinical notes electronically as mandated by the NYSDOH Health Home regulatory requirements
5. Must be flexible with hours based on needs of the population served by Niagara Falls Memorial Medical Center Health Home.
6. Able to manage effectively in difficult situations
